I knew a woman nurse here who joint ventured with a nutritionist & they did joint group therapy sessions for diabetics.

They found their target market by going to independent doctors(not associated with an health
maintenance organization).

The doctors liked having the follow up with their patients.

The service surprisingly enough was paid by the clients.

But for the low income....you might want to consider doing the service as a non profit...

Anyway, hope this helps....
