Sam,
Thanks for taking action.
If you want do add stuff about phone collecting, then I think it would also be wise to have some audio so the customer can Hear how it should sound, besides just reading it.
So visit Wavepad's website at http://www.nch.com.au/wavepad/ and use that, together with a handheld mic available at any Tandy, Dicksmith, RadioShack, Jaycar store, and record into your computer. Then you can burn that file to the CD as well.
How to prevent the debt - or encourage fast payments - would also be a good addition. And there are many tricks and tips to that.
For instance: one thing I did was copied how the phone company puts their due date.
They used to have it like this...

Total Amount Due: $120
Due Date: 33-Sep-2005
When I went to that format, I noticed the speed of payment increased - I got my money quicker.
And now I notice they have changed again to this...

Total $120
Pay by 33 Sep '05

The amount is LARGE and the Pay By line starts where the dollar sign begins. So it looks like the amount and due date are together.
Notice that this new format is a COMMAND. The recipient is being commanded to pay by that date. I like it.
They also have "To avoid a late fee please pay by the due date" a few lines beneath the amount.
There are lot of tricks like these you can use to help encourage fast payment. So yes, I like the idea of adding something like that to the product as well.
